Lost Tooth Form

Our eldest child lost their first wobbly tooth while and nursery, and didn't notice until I was helping them to brush their teeth in the evening. They were then super sad when they didn't have a tooth to leave under their pillow for the tooth fairy. So I created a form we could complete together and put under their pillow instead, as a sort-of joke imagining that tooth fairies also have to deal with paperwork.

I shared this on Mastodon and it turned out to be something people liked, and a number of people asked for a copy of the form, so is an 'official' lost tooth form that a child who as misplaced a lost tooth, to allow the Tooth Fairies to do their usual job. Available as:

Print on A4 and then cut out. Yes, it's small, but so are tooth fairies. Hopefully I've corrected all the spelling mistakes of the first hurried version. From persona experience I can say it works, and the tooth fairy accepted it in instead of a real tooth.

Verlorener Zahn Formular für die Zahnfee

Mein ältestes Kind hat deren ersten Wackelzahn in der Kita verloren, und es garnicht gemerkt. Als er traurig war, das er keinen Zahn für die Zahnfee hatte, habe ich schnell dieses Formula erstellt, dass wir stattdessen unter seinen Kopfkissen gelegt haben. Und es hat geklappt, die Zahnfee ist wie erwartet gekommen! Es ist nur ein Witz, aber hat geholfen, und jeder weiss das in Deutschland geht alles nur mit Papierkram.
